Dit is een herzien antwoord op basis van uw vereisten. Natuurlijk is er geen heilige graal voor schema's, maar ik zou mijn antwoord vóór revisie willen motiveren (het houden van een platte structuur is gemakkelijk te doorzoeken en te onderhouden) met enkele voorbeeldgegevens + query's op basis van uw lijst met vereisten. Ik herhaal, ik zeg niet dat dit de beste oplossing is, maar het is een oplossing die eenvoudig op te vragen en gemakkelijk te onderhouden is (imho).

Toegegeven, het nadeel van mijn voorgestelde oplossing is dat niet alle vereisten in één zijn opgenomen vraag.
